Submitting a New Out-of-Network Service Request (For a Non-lab-based Test)

When requesting a non-laboratory-based test—such as a POCT (Point-of-Care Test) drug test or a breath alcohol test—you’ll complete a slightly different version of the service request form. These tests are typically performed on-site using a designated testing device, with immediate results. This article explains how to submit a New Out-of-Network On-Site Service Request from the TPA dashboard.

1. Employer

Select the Employer (your client) from the dropdown list. This links the service to the correct organization for reporting, compliance, and billing purposes. Only previously added clients will appear in this list.

2. Donor

Choose the Donor who will be tested. The dropdown list will display donors associated with the selected employer. Be sure the donor’s profile is up to date with identifying information. You may add new donors from this page by clicking the "New Donor" button.

3. Test

Select the specific Test you are requesting. This corresponds to the service listed by the provider, such as:

  • 7-Panel Urine POCT
  • 12-Panel Oral Fluid POCT
  • Breath Alcohol Test

This list is populated from the Out-of-Network Other Test Services the provider has made available.

4. Testing Device

Choose the Testing Device that will be used by the provider. This ensures the requester and provider are aligned on the method and equipment. Examples include:

  • VeriCheck 7-Panel Drug Test Cup
  • VeriCheck 11-Panel Oral Fluid Drug Test
  • Alcovisor Mercury EBT
  • Alcoscreen 02 Oral Fluid Alcohol Test

5. Reason for Test

Select the Reason for Test to indicate the purpose behind the screening. Standard options include:

  • Pre-employment
  • Random
  • Post-accident
  • Reasonable suspicion
  • Return-to-duty
  • Follow-up

Finalizing the Request

  1. Review all fields to ensure accuracy and completeness
  2. Click Submit to send the request to the provider
  3. The provider will receive the request in their Out-of-Network Service Requests Received tab and can approve, modify, or decline it

For POCT or breath alcohol tests, results may be delivered more quickly than lab-based tests. Be sure to monitor the status of the request and follow up as needed.
